In Reply We appreciate the thoughtful commentary provided by Dr Schober and colleagues in response to our recent article that documented sudden cardiac arrest and death during US-based long-distance running races from 2010 to 2023. The authors question the accuracy of our cardiac arrest and death incidence rates with a focus on the methodology used to generate the denominator component of these statistics. As described, the denominator used for our incidence estimates relied on the total number of race finishers, a metric carefully tabulated on an annual basis by our research partner Running USA. However, the total number of race finishers during the study period may have included repeat race finishers. As transparently acknowledged in the article, we concur with Schober and colleagues that this introduced the possibility that our incidence data may have underestimated the true “per-person” risk of cardiac arrest and death.
