To the Editor The COPDGene 2025 Diagnosis Working Group and the CanCOLD Investigators proposed a multidimensional diagnostic approach for COPD that includes a category without evidence for obstruction. In an accompanying Editorial, Dr Polverino asked “…an even deeper question: if airflow obstruction is no longer essential for diagnosis, should we still be calling it COPD?” More than 50 years ago, the term chronic nonspecific lung disease was proposed as an umbrella term grouping chronic bronchitis, asthma, emphysema, and irreversible or persistent obstructive lung disease. The COPDGene 2025 Diagnosis Working Group and CanCOLD Investigators acknowledged substantial clinicopathologic heterogeneity within the current rubric “COPD.” Similar heterogeneity exists within the rubric “asthma.” These clinicopathologic heterogeneities are incorporated into the new paradigm for treating chronic airway diseases, so-called treatable traits, that proposes to personalize treatment based on individual biology, not diagnostic categories. I propose an even deeper answer to the deep question posed earlier: call it chronic nonspecific lung disease.
