To the Editor A recent randomized clinical trial evaluating adjunctive intrawound tobramycin in periarticular tibial fractures reported lack of benefit with combination therapy, which is notable, particularly given the common assumption that broader or higher local antibiotic exposure should translate into improved infection prevention.
